Step 1: Schedule your appointment AND Fill out intake forms
Whether you schedule via phone call, online scheduling tool, email, or form - the first step will be to have you fill out some paperwork online through our patient portal so that we can get to know you and your goals prior to your initial visit.
Step 2: meet the provider
Once the intake forms are filled out, your provider will review these forms and may schedule a brief call if there are any additional questions that we may have for you. The reason for this is to have a brief consultation so that we can make sure that we will be able to help you. You will not be charged for this consultation, and if the provider believes that your complaint would be better served by another professional, he will refer you to them. This consultation typically takes less than 10 minutes and can be performed on the same day as your first appointment if needed.
Step 3: YoUr First Visit
We will begin by reviewing the complete history of your symptoms, why you are looking to receive care, and goals for treatment. An exam will follow this thorough consultation to rule out any red flags or contraindications to treatment and determine exactly what is causing your symptoms (muscle, nerve, tendon, joint, etc.). Once the exam is completed, treatment begins. Treatment will be 30-90 minutes long, consisting of a combination (some or all) of muscle work, joint manipulations (adjustments), stretching, cupping, therapeutic exercises, and more (see services for full list).
Step 4: Visit Follow Up
Following your first visit, you will receive all of the necessary information regarding your care within 24 hours. Action items, exercise programs, care plans, at home recommendations, and anything else discussed will be shared with you. If you are submitting to your insurance for reimbursement, you will also receive a copy of your superbill.
Now that you are officially a member of Innove Chiro & Rehab, you will have easy access to your provider to continue to track your progress and to answer any follow-up questions.
Step 5: Future Visits
This experience applies to all future visits. At each visit you will continue to get the same quality of care and focused attention as your first. Depending on your condition and goals, your home corrective exercise plan and other recommendations will be updated throughout care, depending on what phase you are in.
